Protected health information
This website is not a channel for protected health information. Please do not submit PHI through the contact form. PHI is exchanged only through secure, contracted channels governed by a Business Associate Agreement.
Data retention and security
Inquiry records are retained only as long as needed for business purposes. Data is encrypted in transit and at rest, with role-based access limited to staff who need it.
